WHAT IS NEEDED TO PROCEED
 

• DOWNLOAD BIRTH FORM or DOWNLOAD DEATH FORM, complete and SIGN form.
  Texas residents can only order:
  - Their own birth records; or
  - The birth records of immediate family members.
  (Qualified applicants include by blood or legal marriage: grandparents, parents, children, brothers and sisters.)
  Non-Texas residents can only order:
  - Their own records; or
  - The records of their children if they are listed as a parent on the record.

• DOWNLOAD and SIGN RSVPpassports Letter of Release.

• Proof of Identity
  You will need one of the following documents and it must contain the applicants name and signature and/or an identifiable photo of the applicant. Document must be valid and current.
  - U.S. issued driver’s license
  - U.S. state issued ID
  - U.S. military ID
  - Permanent resident card or green card
  - U.S. passport
  - Visa
